Every patient and staff of Evergreen is expected to complete following pre-screening questionnaire at least 2 hrs before in-person appointment.

EVERGREEN RE-OPENING (MAY 19 2020) PROTOCOLS
Dear patients of Evergreen,
We are thrilled to announce that we will be re-opening our two clinics starting Tuesday, May 19, 2020 after long weekend. Our primary goal at this point to provide safe and healthy environment for our patients and staff. To do this, we have implemented several important changes to our usual way of doing business. Evergreen Rehab & Wellness is to fully comply the approved guidance from the Public Health officer, WorkSafeBC, BC Centre for Disease Control (BCCDC) and the each of our therapists’ respective governing body guideline (Chiropractic/Physiotherapy/RMT/Acupuncture etc) to resume in-person services, surrounding best practices to ensure a safe and gradual resumption to in-clinic care.
Please understand the implementation of following protocols. These will be our “New Normal” until the pandemic is over.
COVID-19 pre-screening should be completed prior to each in-person appointment. Written and verbal consent will be in place.
Please wear mask or face-coverings while inside the clinic.
Patients are asked to come alone to appointment to avoid congestion in the reception and waiting room area. We will remove most chairs out of the waiting room. If you need a guardian to assist you, please call to clinic in advance.
We need every patient to be familiar with our online system. We will be sending out consent forms via JaneApp system. In order to reduce congestion and traffic in the waiting room, we ask all new patients to fill out their intake forms online prior to their appointment.
Please arrive early for your appointment. Please wait outside until we call you to come in.
We mandate hand sanitizing or hand washing before and after treatment. We created few hand sanitizing stations in the clinic for your convenience.
Please come the pre-screening questionnaire (which will be sent via email and can be completed in JaneApp system) 2 hr prior to your appointment. You are expected to complete it for every appointment going forward, and we will require your written or verbal consent for it. If you have any symptoms, please call to office.
We prepared a touch-less payment processing system (Tap function). The patients may also leave their credit card information which will be securely stored in our JaneApp system and streamline the payment process. We will not accept cash payment for the time being.
Please understand if our receptionists having difficulty answering your call as we will be getting back to operation with limited capacity. If you need an appointment, please use our online booking page for appointment.
Various clinic safety measures are currently in place for both Coquitlam and Surrey Office. We installed plexiglass barriers in our reception to minimize transmission of virus by droplets. We are using medical grade disinfectant cleaners and sanitization products approved by Health Canada. We have increased time between appointments to reduce social contact and allow time for through sanitization. We also advise and train patients and staffs to minimize any congestion in high traffic zone in the clinic. As written above, we have implemented pre-screening for COVID 19 mandatory for all patients and staffs.
